Support Worker to provide person-centred support to people with disability to assist with daily living, personal care, mobility, community access and maintaining independence.
The role involves providing safe, respectful and reliable support in accordance with the individual’s support plan, preferences, goals and needs.
Key Responsibilities, Personal Care & Daily Living
- Assist with personal care, hygiene, dressing and grooming.
- Support with toileting and continence management where required.
- Assist with household and daily living tasks.
Mobility & Transfers
- Assist with safe transfers and mobility.
- Previous experience using hoists and transfer equipment is essential.
- Safely operate and use mobility and transfer equipment in accordance with training and procedures.
- Follow manual handling and safety procedures.
- Report any falls, injuries, hazards or changes in mobility.
Health, Safety & Wellbeing
- Follow individual support plans, risk assessments and relevant procedures.
- Monitor and report changes in wellbeing or support needs.
- Assist with maintaining personal hygiene and skin integrity.
- Maintain a safe and clean environment.
- Complete incident reports where required.
Documentation & Communication
- Complete accurate case notes and shift documentation.
- Communicate relevant information to the appropriate supervisor or coordinator.
- Maintain confidentiality and professional boundaries.
- Follow all organisational policies and procedures.
Essential Requirements
- Previous experience in disability support or a similar care role.
- Demonstrated experience using hoists and transfer equipment is essential.
- Good communication and interpersonal skills.
- Ability to provide respectful and person-centred support.
- Ability to follow support plans and safety procedures.
- Ability to work independently and as part of a team.
- Current NDIS Worker Screening Check.
- Current First Aid and CPR.
- Relevant checks and training as required for the role
Pay: $40.00 – $60.00 per hour
Expected hours: No less than 15.0 per week
Work Location: In person
