About the role:
The Pharmacist for Ambulatory Care will provide a key role in a modern, hybrid model of care that blends Hospital in the Home (HITH), Aged Care Outreach Service (ACOS), Procedures, Investigations and Infusions (PIXI) services. As a pharmacist within this service, you will empower equitable service delivery of care by utilising outreach into Residential Aged Care Homes (RACH), increasing virtual care modalities and coordinating escalation pathways for inpatient and emergency services.
Provide clinical pharmacy services within a Health Service Facility, including medication provision and supply, medication reconciliation, pharmaceutical reviews, patient and staff education, and support of patients and clinical teams as an active member of the multidisciplinary healthcare team.

Our ideal candidate will demonstrate:
Registration as a Pharmacist authorised to practise by the Australian Health Practitioner Regulation Agency (AHPRA).
Experience and skills in clinical pharmacy with the ability to apply this knowledge to practical situations.
Knowledge of legislation governing drug use, in both hospital and community settings; understanding of medication safety, quality assurance, PBS and best practise.
Proven organisational and time management skills and with the ability to prioritise according to organisational needs.
Strong communication skills and the ability to engage effectively with patients, carers and healthcare professionals.
