Your new role:
As the Aboriginal and Torres Strait Islander Nurse Navigator, you'll coordinate culturally safe, person-centred care for Aboriginal and Torres Strait Islander patients with complex health needs. Working across hospital and community settings, you'll partner with patients, families and multidisciplinary teams to improve health outcomes and advance health equity.
In this role you will:
Coordinate complex care across primary, community and hospital services to deliver seamless patient journeys.
Build strong partnerships with patients, families, communities and multidisciplinary teams to provide culturally safe, holistic care.
Lead service improvements through clinical leadership, education, mentoring and quality initiatives.
Advocate for better health outcomes by empowering patients and supporting innovative, integrated models of care.
This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
About you:
We're looking for an experienced Aboriginal and/or Torres Strait Islander Registered Nurse who is passionate about improving health outcomes for First Nations peoples through compassionate care, strong partnerships and clinical leadership.
You'll bring:
Advanced clinical nursing experience with the ability to coordinate care for patients with complex health needs.
Strong knowledge of Aboriginal and Torres Strait Islander health and a commitment to delivering culturally safe, person-centred care.
Excellent communication, relationship-building and leadership skills, with the ability to collaborate across healthcare settings.
Sound clinical judgement, evidence-based practice and a passion for improving health equity through innovation and continuous improvement.

The Metro South Health First Nations Health Directorate is responsible for providing support, guidance and feedback on improvement strategies relating to Department of Health - First Nations Health Equity Strategies: Co-development and Co-implementation to Metro South Hospital and Health Service; Queensland Health Aboriginal and Torres Strait Islander Cultural Capability Framework 2010-2033; and Metro South Health - Strategic Plan 2024-2085 and Integrated Planning Framework 2023 Revision; and the Metro South Health - First Nations Health Equity Strategy 2022-2025.
