About the role
Provide high-quality Social Work services as part of the Geriatric Evaluation and Management Service (GEMS) at Brighton Health Campus. Working within a multidisciplinary team, you will deliver assessment, intervention, evaluation, and case management services that support older patients experiencing complex medical conditions, frailty, chronic disease, and functional decline.
You will contribute to the delivery of person-centred, restorative care that promotes independence, wellbeing, and quality of life for older people. The role involves working closely with patients, families, carers, and healthcare professionals to support effective care planning, discharge coordination, and access to appropriate services, while contributing to evidence-based practice and service improvement initiatives.

Deliver professional Social Work assessment, intervention, case management, and support services within a multidisciplinary team environment
