About the role
Based at our Mirrabooka office, this permanent full-time position plays a vital role within our Intensive Family Support Service, working alongside families who are currently engaged with the Department of Communities within the Mirrabooka and Joondalup regions.
You will build trusted relationships with families, undertake comprehensive assessments, develop tailored support plans, and provide intensive case management to help families achieve greater stability and wellbeing. Through a trauma-informed and strengths-based approach, you will empower families to navigate complex systems, access services, strengthen protective factors, and work towards their safety goals with the Department of Communities.
This role offers a diverse mix of direct client work, advocacy, stakeholder engagement, community collaboration, and case coordination. You will work closely with internal and external service providers to ensure families receive the support they need and maintain a positive working partnership with the Department of Communities to support with contributing to positive long-term outcomes for children, young people and their families.
About you
You are a compassionate and resilient practitioner who enjoys building meaningful relationships and supporting people through complex challenges. You bring a person-centred approach, strong professional judgement, and the ability to balance autonomy with collaboration.
In addition, you will also possess:
- A relevant Bachelor's degree in Social Work, Psychology, Community Services or a related discipline (preferred)
- Experience delivering case management and evidence-based interventions within community services, family support, health, or related sectors
- Strong understanding of trauma-informed, strengths-based and culturally responsive practice
- Excellent communication, relationship-building and stakeholder engagement skills
- The ability to manage competing priorities, maintain accurate records and work both independently and within a team
- Current Driver's Licence
- Working With Children Check (WWCC)
- National Police Clearance (obtained within the last six months)
