Key Responsibilities include:
-assessing the social, psychosocial and support needs of clients, their families and carers in home and community settings
-conducting individual and family case interviews to identify the nature and extent of a client's circumstances, risks and support requirements
-developing, implementing and reviewing individualised care and support plans that reflect each client's goals, choices and cultural needs
-identifying risk factors including social isolation, abuse, neglect, financial hardship and carer stress, and putting appropriate safeguards in place
-assisting clients to understand and resolve personal, social and practical problems through information, support and advocacy
-acting as a facilitator and mediator between clients, families, carers, community services and service providers
-supporting clients and their families through periods of transition, including hospital discharge, changes in living arrangements and end-of-life care
-referring and linking clients to specialist services and coordinating multidisciplinary input from general practitioners, allied health professionals, hospitals and community organisations
-monitoring client progress through home visits, telephone contact and review meetings, and maintaining accurate case records, assessments and progress notes
-preparing reports, submissions and documentation for funding bodies, regulators and other stakeholders
-providing guidance, education and secondary consultation to care staff and support workers on psychosocial matters
-contributing to the development and review of policies, procedures and practice guidelines, and identifying unmet community needs
Requirements:
A bachelor degree or higher qualification in Social Work.
Experience within a community/home care/residential care/disability care setting desired.
Outstanding interpersonal and communication skills.
Ability to commute to client homes.
Valid driver's license and access to dependable transportation.
Police Check.
Up to date with Flu & COVID-19 Vaccinations.
