About the role and where you will be working:
We have a great opportunity for an experienced Therapy Assistant to work in Community Transition Care Program (TCP) in the South West Metro area as a part of our Home Care team.
As a Southern Cross Care Home Care Therapy Assistant, you will have a passion for working in partnership with older people and for delivering person centred evidence-based therapy. You will maintain an active case load and engage with our care teams supporting our clients to achieve their physical and activity goals.
Your responsibilities will include but not limited to:
Support allied health professionals to deliver therapy programs that promote independence, mobility and wellbeing in the home
Assist clients with exercises, daily living activities and mobility programs, monitoring progress and reporting changes
Build positive relationships with clients to encourage engagement and achievement of individual goals
Help set up equipment and minor home modifications, providing guidance on safe use of aids and supports
Maintain accurate client records and communicate updates with the wider care team to support continuity of care
Identify and report risks or hazards, while working safely, following clinical direction and staying within scope of practice
About you:

Willingness to embrace the values of Southern Cross Care WA and rewarding industry and how it plays into the role
Passionate about helping people stay independent and achieve meaningful outcomes in their own homes
You bring relevant qualifications (Cert III or IV in Allied Health Assistance or similar) and enjoy working in aged care, disability or community settings
Strong communicator who builds trust easily and supports clients with empathy and respect
Organised and self-driven, comfortable managing your day and balancing a mobile workload
Confident using technology and take a proactive, safety-first approach in everything you do
To be considered for this role you’ll need:

2 years experience working as a Therapy Assistant is preferred, however previous work in a community setting will be considered.

Ability to work collaboratively within the TCP team and deliver goal focused interventions, to improve clients independence and function.

A current Australian Driver’s Licence
-
You will also need a strong commitment to person centred care and able to work both independently and within a team. Most importantly, you'll have a genuine desire to make a positive difference in our clients’ lives.
