Your new role:
As a Clinical Nurse within the Dementia Outreach Service, you will play a pivotal role in delivering specialist dementia nursing care across residential aged care facilities and the community. Working as part of a multidisciplinary team, you will provide advanced clinical expertise while supporting best practice, education and service improvement.
Provide advanced assessment, care planning and specialist clinical management for people living with dementia and complex care needs.
Deliver clinical leadership by mentoring staff, supporting evidence-based practice, and contributing to education, quality improvement and service development.
Collaborate with multidisciplinary teams, residential aged care providers and community partners to achieve person-centred outcomes and reduce avoidable hospital presentations.
About you:
ou are a compassionate and experienced Registered Nurse with a passion for improving the lives of people living with dementia. You thrive in collaborative environments, enjoy leading others and are committed to delivering exceptional, person-centred care.
Demonstrated advanced clinical knowledge and experience in dementia care, including assessment and management of people with cognitive impairment and complex care needs.
Proven ability to provide clinical leadership, build capability in others and foster strong collaborative relationships across multidisciplinary teams.
Highly developed critical thinking, communication and problem-solving skills, with the ability to work autonomously and manage competing priorities.
