What you’ll be doing
As a Senior Social Worker, you’ll play a pivotal role in supporting clients and families through their rehabilitation journey. You will provide advanced clinical expertise and leadership across the Acute Stroke Unit and Geriatric Medicine services. Working with a high degree of autonomy, you'll manage complex psychosocial presentations while supporting patients, families and the multidisciplinary team to achieve the best possible outcomes.
In this role you will be:
Delivering expert psychosocial assessment, counselling, advocacy, crisis intervention and discharge planning for patients with complex health and social needs.
Providing clinical leadership, consultation and specialist advice across Acute Stroke, Rehabilitation and Geriatric Medicine services.
Partnering with medical, nursing and allied health teams to coordinate safe, high-quality patient care in complex and high-risk situations.
Mentoring and supervising social workers and students while contributing to education, clinical supervision and evidence-based practice initiatives.
Supporting service development through quality improvement, policy implementation and continuous evaluation of clinical services.

Demonstrated experience in staff and student supervision,
Demonstrated high level clinical experience in social work assessments, crisis intervention and counselling patients and families, with relevant demonstrated skills for matters such as coping with illnesses, Elder abuse, Guardianship matters, NDIS matters, grief and bereavement, accommodation placements and complex familial presentations.
Experience in developing and implementing policies and procedures for service improvement and skills in providing training and education.
Demonstrated well-developed time management skills with excellent written and verbal communication, including report writing skills.
Demonstrated Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stake holders throughout all organisational levels.
Demonstrated Ability to initiate, lead, complete and implement quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff.
Unrestricted NSW Driver’s Licence and ability to participate in the After Hours Crisis On-Call Social Work Service.
