Provide leadership in patient-centred assessment, care coordination, and follow-up within a well-established Osteoporosis Re-Fracture Prevention (ORP) program aligned to the ACI Model of Care.

Collaborate closely with a supportive multidisciplinary team, including Rheumatology, Orthopaedics, Aged Care, and community providers.

Apply advanced clinical judgement to identify patients at high risk of fracture and discuss osteoporosis medication options to reduce fracture risk.

Contribute to service development, quality improvement initiatives, and data-driven evaluation, including patient-reported outcomes.

Engage in research and innovation within a growing chronic disease and musculoskeletal service.

Bring substantial clinical experience managing patients with chronic or complex conditions, ideally within musculoskeletal, orthopaedic, geriatric, or acute care settings.

Demonstrate the ability to independently manage complex caseloads, apply advanced clinical judgement, and deliver evidence-based care in a fast-paced environment.

Exhibit strong communication and relationship-building skills with patients, carers, and multidisciplinary teams, with a commitment to person-centred care.

Have experience in chronic disease management and care coordination, with the ability to develop and implement collaborative care pathways across acute and community services.

Apply strong analytical and organisational skills, including interpreting clinical data, contributing to quality improvement, confidently using clinical systems, and working both independently and as part of a supportive team.
