Your new role:
As the Clinical Nurse Consultant - Mental Health Clinical Care Coordinator, you will provide advanced clinical leadership and consultancy to support seamless care pathways for people experiencing suicidal crisis or who have attempted suicide. Working within the Acute Care Team, you will collaborate with multidisciplinary teams and the non-government service provider to strengthen referral pathways, care coordination and recovery-focused outcomes.
In this role, you will:
Provide advanced clinical expertise and leadership in mental health nursing practice.
Coordinate referrals and care delivery between Metro South Health and The Way Back Support Service.
Provide clinical consultancy, education, mentoring and support to nurses and multidisciplinary teams.
Lead quality improvement, evidence-based practice and service development initiatives.
About you:
We are looking for an experienced mental health nurse who can demonstrate advanced clinical knowledge, sound professional judgement and strong leadership skills. You will be confident working collaboratively across clinical and community settings and building effective relationships with internal and external partners.
You will bring:
Advanced mental health nursing knowledge, assessment and clinical decision-making skills.
Strong communication, negotiation and relationship-building skills.
Demonstrated ability to coordinate complex care and develop effective referral and escalation pathways.
A commitment to evidence-based practice, quality improvement, education and positive patient outcomes.
