HPS is seeking a passionate and adaptable Clinical Pharmacist to support healthcare services across a diverse correctional health network in Victoria. This unique role offers the opportunity to work closely with multidisciplinary healthcare teams, providing medication management and clinical pharmacy services to a complex patient population.
This is an ideal opportunity for a pharmacist who enjoys autonomy, clinical problem-solving and building strong relationships with healthcare providers while contributing to meaningful health outcomes.

What You'll Be Doing
Conduct clinical medication reviews and medication reconciliation
Provide expert medication advice to medical, nursing and healthcare teams
Support safe and effective use of high-risk and controlled medicines
Identify opportunities to improve medication safety and patient outcomes
Participate in clinical governance and quality improvement activities
Build strong partnerships with multidisciplinary healthcare stakeholders
Weekly Work Locations
Monday – Middleton & Loddon Correctional Centres, Castlemaine
Tuesday – Malmsbury Youth Justice Precinct, Malmsbury
Wednesday – Western Plains Correctional Centre, Lara
Thursday – Melbourne Remand Centre, Ravenhall
Friday – Dame Phyllis Frost Centre, Ravenhall
Travel allowance payable for regional locations (Castlemaine, Malmsbury and Lara) on a pro-rata basis.
About You
Current AHPRA registration as a Pharmacist
Strong clinical knowledge and patient-centred approach
Excellent communication and stakeholder engagement skills
Ability to work independently across multiple locations
Strong organisational and problem-solving skills
