About the role
Clinical Nurse Specialist – Grade 2 Discharge Planner is responsible for coordinating and facilitating safe, timely, and effective discharge planning for patients across the healthcare continuum. Working collaboratively with patients, families, carers, multidisciplinary teams, and external service providers, the position ensures patients experience seamless transitions from hospital to home, residential care, rehabilitation, community services, or other healthcare settings. The role supports efficient patient flow, reduces avoidable delays in discharge, minimises readmissions, and promotes positive patient outcomes through patient-centred discharge planning and care coordination.
