Care Advisor – Support at Home

We’re looking for a compassionate and experienced Care Advisor to join our Support at Home team on a full-time basis. In this rewarding role, you'll work alongside older people, their families and carers to coordinate services that help clients remain safe, independent and connected to their community for as long as possible.

About the role
As a Care Advisor, you'll provide care coordination and ongoing support to older people receiving Support at Home services. You'll work closely with clients, families, service providers and clinical teams to ensure services are responsive, person-centred and aligned with each client's goals and changing needs.
Your responsibilities will include:
- Building positive and trusting relationships with clients, families and carers.
- Supporting clients to understand available services and navigate the aged care system.
- Coordinating support services and monitoring service delivery to ensure clients receive timely and appropriate care.
- Managing an allocated caseload and conducting regular client reviews.
- Monitoring client wellbeing, identifying changes in circumstances and escalating concerns where required.
- Liaising with service providers, health professionals and community supports to deliver positive client outcomes.
- Monitoring service budgets and supporting clients to understand funding and service options.
- Maintaining accurate documentation and ensuring compliance with aged care standards and organisational requirements.
About you
You'll be someone who enjoys helping older people achieve their goals and remain living independently at home. You'll be experienced in this role, have strong relationship-building skills, excellent organisational abilities and a genuine commitment to providing high-quality client care.
To be successful, you'll bring:
- A Certificate IV or Diploma in Community Services, Ageing Support, Disability, Diploma of Nursing (Enrolled Nurse), or equivalent experience.
- Experience working in aged care, community care or case management.
- Experience supporting older people living in the community, ideally within Home Care Packages or Support at Home programs.
- Knowledge of the aged care sector and Support at Home program.
- Strong communication, problem-solving and relationship-management skills.
- Experience managing service budgets and coordinating supports.
- The ability to work independently while contributing positively to a team environment.
- Strong computer skills, with experience using aged care systems viewed favourably.

Location
This role is primarily based in St Kilda, with occasional travel to other BHN locations and client homes as required.
