The Role:
We are seeking an experienced, enthusiastic, and highly motivated In-Home Care Coordinator to join our team in Campbelltown. This role is pivotal in managing, coordinating, and delivering high-quality, person-centered care to our diverse client base.
Key Responsibilities:
- Conduct comprehensive Client Care Assessments and develop tailored Care Plans aligned with individual needs and goals.
- Manage a caseload of clients, ensuring compliance with care standards and funding requirements.
- Handle new client inquiries professionally and efficiently over the phone, aiming to schedule care consultations promptly.
- Coordinate the allocation of in-home care services based on client needs, staff availability, geographical considerations, and funding constraints.
- Oversee the delivery of person-centered, health, personal care, and social activities, ensuring they are in line with documented plans, including health care, mental health, and behavior support plans.
- Supervise, support, and guide support workers to ensure high standards of service delivery.
- Assist with general administrative tasks such as scheduling, documentation, and maintaining client records.
- Contribute to staff recruitment, training, and development processes.
- Develop and grow the service footprint within the South West and Western Sydney regions, fostering strong community relationships.
- Maintain professional communication with clients, families, support workers, and external agencies to ensure holistic support and service satisfaction.
What You’ll Need
- Degree in Nursing, Social Work, or related field (or equivalent experience)
- Minimum 3 years’ experience in aged care or community care coordination
- Current Driver’s Licence and insured vehicleValid First Aid & CPR Certificate
- Cleared National Criminal History Check and WWCC (or willing to obtain)
