Band 6 - Salary Range $97,926 to $106,637 per annum + 12% Super + ADO

The Opportunity
An exciting fixed-term, full-time opportunity within our Community Care & Active Living, Client Connections team for an enthusiastic, innovative and motivated leader.
Reporting to the Client Connections Team Leader, you will lead and support a dedicated team delivering high-quality services, including Domestic Assistance, Personal Care, Flexible Respite and Individual Social Support, to older people, people with disability and their carers.

Key Responsibilities
- Lead, coach and develop a team of Community Care Workers to deliver high-quality, person-centred care and achieve positive client outcomes.
- Coordinate client referrals and oversee the delivery of services funded through the Commonwealth Home Support Programme (CHSP) and Home and Community Care Program for Younger People (HACC PYP), ensuring timely, responsive and effective support for clients.
- Foster collaborative relationships with clients, carers and key stakeholders to deliver tailored services that promote independence and wellbeing.
- Lead continuous improvement initiatives and ensure services are delivered in accordance with legislative requirements, funding agreements, Council policies and Aged Care standards.
- Contribute to workforce planning, resource management and service performance to ensure sustainable, efficient and effective service delivery.

About you
To be successful in this role you will have the following:
- Tertiary qualification in health, community services or related field and some relevant experience or lesser formal qualifications with substantial experience in a similar position.
- Experience delivering CHSP and HACC PYP services within a community care or aged care setting.
- Current First Aid and CPR Certificates.
- Current valid Victorian Driver’s Licence.
- Must hold or be able to obtain an Employee Working with Children Check.
