What would I be doing?
As a Service Facilitator, your role will be to facilitate the provision of high-quality and individualised services to people with disability, in line with the person’s choice, preferences and support plans.
This will include:

Ensuring the provision of high-quality services and support to people with disability with NDIS and other funding, and in line with regulatory & organisational guidelines.

Liaising with the client, and external stakeholders to ensure appropriate access to general and specialist health care, allied health support and ensuring all health care plans are implemented effectively.

Proactive and supportive client and family engagement to support client decision making and choice and ensuring excellent communication with each client’s informal networks.

Focusing on continued improvement to enhance the service experience for each client and family.

Ensuring the health and wellbeing of clients and their team of direct support workers and contributing to a welcoming, diverse and inclusive workplace.
- Developing client rosters and strong service teams to provide exceptional care in line with client support needs and funding plans.

Assisting clients to manage their own budgets and support transparent accountability of the use of personal funds.

Leading a team of direct support workers through supervision, training, coaching and mentoring.

Effectively communicating with and coordinating services, staff and stakeholders who provide support to clients.

What you’ll need

A commitment to the highest level of customer service through facilitating quality disability support services and complex supports needs.

Pro-active, energetic approach to a dynamic ever-changing environment including knowledge of the NDIS.

Experience and skill in team supervision, leading people to achieve.

Highly effective interpersonal and communication skills enhanced by well-developed administrative skills.

Able to work autonomously and collaboratively within teams.
-
Willingness to be part of a rotating back up On Call roster.

A commitment to working together to deliver great outcomes.
- Great listening and respectful communication skills.

Competency with Microsoft Office and computer systems.

Blue Card & NDIS Worker Screening Check (we can help you get these).

Current First Aid & CPR certificates.
