What you'll be doing
As a Clinical Nurse Specialist, you will provide advanced clinical leadership and specialised nursing care to patients with complex supportive and palliative care needs. You will play a key role in coordinating care across settings, supporting patients and carers, and ensuring care aligns with individual goals and preferences.
In this role, you will:
- Coordinate and manage complex palliative care plans across the patient journey
- Lead evidence-based clinical practice and quality improvement initiatives
- Provide expert clinical advice, mentorship and support to nursing staff and multidisciplinary teams
- Facilitate seamless transitions of care between hospital, community and home-based services
- Promote patient-centred care that respects dignity, choice and end-of-life preferences

Essential Criteria:
- Current Registration as a Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA)
Selection Criteria:
1. Demonstrated at least three ( years full time equivalent post registration experience in Palliative Care, Cancer Care Services, Advanced Nursing or clinical experience deemed appropriate by the employer
2. Post graduate qualifications in Palliative Care, Cancer Care or Advanced Nursing
3. Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning
4. Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Policies and National and State Standards related to Palliative Care
5. Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations
6. Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based service
7. Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
