Position Purpose
The Registered Nurse provides safe, person-centred and culturally responsive nursing care to older people receiving Support at Home services, supporting them to remain safely and independently in their own homes.
Key Responsibilities
- Conduct comprehensive nursing assessments and identify clinical and environmental risks.
- Develop, implement and regularly review individualised care plans.
- Monitor consumers' health and respond appropriately to changes or deterioration.
- Provide clinical advice, health education and nursing care within scope of practice.
- Coordinate care with GPs, allied health professionals, support workers, families and other providers.
- Support medication, wound, continence, nutrition, chronic disease and palliative care needs as appropriate.
- Maintain accurate clinical records and documentation.
- Report and manage incidents, risks, complaints and safeguarding concerns.
- Provide clinical guidance to support workers where required.
- Participate in clinical audits, quality improvement and compliance activities.
- Promote dignity, choice, independence, privacy and culturally appropriate care.
Essential Requirements
- Current AHPRA Registered Nurse registration.
- Relevant aged care, community or clinical nursing experience.
- Strong clinical assessment, communication and documentation skills.
- Ability to work independently in a community/home-care environment.
- Current police/background screening and required vaccinations/checks.
- Current First Aid/CPR and driver's licence where required.
