Care Partner –Support at Home and CHSP
PAWA Community Care Inc is looking for an experienced Care Partner to join our team.

Key responsibilities
As a Care Partner you will support and work with participants and other stakeholders focusing on their specific needs and goals to maximize their choices and independence. You will develop and review quality care plans, needs, ensure efficient service delivery within a person-cantered, wellness & enablement approach within legislative and regulatory requirements of the SAH and CHSP programs, strengthen aged care standards, and other relevant legal frameworks.
What will you be doing?
- On-Boarding new participants – Support at Home and CHSP; and assessing their needs and services required
- Ensuring all participant documentation is complete, correct and forwarded to the appropriate area for filing
- Assessing participant’s needs, developing, monitoring and reviewing Person Centred Care Plans for all participants
- Assisting participants by referring them to providers that can provide a different range of services and liaising with these providers such as: Allied health providers, Australian Government Organisations, community groups and agencies and private organisations to enhance delivery of our community services.
- Ensuring PAWACC develops and maintains a supportive and positive relationship with participants and their families and advocates.
- Working with Support at Home to ensure that they reach maximum benefit from the SAH subsidy including guidance and assistance with eligible expenditure of their funding.
- Managing participant’s budgets and preparing reports and submissions for reviews for funding level upgrades and support review plans.
- Working with participants to promote independence and reablement.
- Managing one to one relationship with participants and their registered supporters
- Providing information and education to participants, families and advocates about available services including government funded services.
- Approving appropriate participant requests.
- Working collaboratively within PAWACC to ensure continuous improvement.
- Establishing appropriate links with the Community, other stakeholders
- Provide support and leadership to Support Workers, including performance management and skills development.
What skills and experience you will bring?
- At least 12 months demonstrated experience in:
o care management in HCP/ SAH and/or CHSP
o development of tailored care plans that support clients to live independently and confidently at home
- Proven knowledge of consumer-directed care and understanding of community aged care services and their requirements.
- Excellent communication, stakeholder engagement, and relationship-building skills
- Sound knowledge of the Aged Care Quality Standards and compliance requirements
- Ability to coach and support frontline staff
- Strong organisational skills with the ability to manage competing priorities
- A client-focused approach with excellent problem-solving and decision-making abilities
What will you need?
· Tertiary qualifications in social work, aged care, allied health or demonstrated competence/ experience.
· Current National Criminal History Record Check
· Current NSW driver’s license and have access to a car for work with the ability to travel regularly.
· Entitlement to work in Australia
· Current First Aid Certificate
Work environment
· You will be expected to travel to participants homes. Must be confident to drive within the Greater Sydney region
· Possibility to apply for hybrid work from office/home after probationary period
· Rostered after hours on call
