As a Clinical Nurse in Community Health, you'll play a vital role in improving health outcomes for people across the community. From delivering advanced clinical care to supporting prevention, education and wellbeing, you'll work with patients, families and multidisciplinary colleagues to make care more accessible, responsive and person-centred.
Every day, you'll contribute by:
Conducting comprehensive assessments and coordinating care that supports people to achieve better health and wellbeing outcomes.
Building trusted partnerships with patients, families and healthcare professionals to deliver connected, holistic care.
Leading safe, evidence-informed practice while identifying opportunities to improve quality, service delivery and patient experiences.
Mentoring and supporting colleagues, students and team members to strengthen capability and foster a positive team culture.

You're a confident and compassionate nurse who combines clinical expertise with genuine care for people and communities. You bring integrity to decision-making, dignity to every interaction and courage when responding to complex situations. Here's what will help you thrive in this role:
Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
Advanced assessment, care planning and care coordination skills in a community or multidisciplinary setting.
Strong communication and relationship-building skills, with the ability to work collaboratively across teams and services.
A commitment to safe, evidence-based practice, continuous improvement and professional development.
You'll stand out from the crowd if you have rural or community nursing experience and/or additional qualifications in areas such as immunisation, rural and isolated practice, or postgraduate study.
Above all, you'll bring Compassion, Integrity, Dignity, Innovation and Courage to every patient, family and colleague interaction.
