The Junction Works is looking for a Support Coordinator to support NDIS Participants and their families and carers to understand, optimise and implement the funded supports in their NDIS plan and to link them to informal, mainstream, and funded supports.
About The Role
The Support Coordinator’s role is one of capacity building to enable participants to have greater choice and control in their lives.
Support coordinators will ensure service agreements and service bookings are completed and can assist in preparing a participant for their NDIS Plan review.
The Support Coordinator needs to understand how to provide person centered support and act with respect for individual rights to freedom of expression, self-determination and decision making.
Key Responsibilities
- Manage a portfolio of participants, completing agreed activities within the hours and budget articulated in customers’ NDIS Plans
- Ensure reporting data is captured and communicated in accordance with requirements set out in the NDIS plan.
- Demonstrate a positive and caring attitude to people we support and their families and carers.
- Apply the NDIS Code of Conduct to all work practices and actions.
- Assist participants to connect and engage with informal and mainstream funded supports and services
Who We’re Looking For
- Experience working with vulnerable people in times of crisis
- Experience working with people living with a disability who may require additional support with their communication or cultural needs
- Excellent verbal and written communication skills
- An understanding and awareness of conflict of interest. In this instance this means support participants to make their own choices and never making decisions on the participant’s behalf.
- Ability to identify and understand challenges facing people from culturally and linguistically diverse and Aboriginal and Torres Strait Islander backgrounds.
- High level of computer skills including a strong understanding of Microsoft Office or 360.
- Demonstrated ability to self-motivate and work in the field, often alone – working directly with participants and their families.
- Tertiary qualifications in case management, community services or other related field is desirable.
