Care Integrity is seeking an experienced and passionate Registered Nurse to join our team as a Care Partner. This role combines clinical expertise with participant-focused care coordination, ensuring individuals receive the right supports to meet their health, wellbeing, and lifestyle goals.
As a Care Partner, you will build strong relationships with participants, families, and support teams while providing clinical oversight, care planning, and guidance to ensure quality service delivery.
Key Responsibilities
- Build and maintain positive relationships with NDIS participants and their support networks.
- Conduct participant assessments and contribute to individualized support and care planning.
- Provide clinical advice and oversight for participants with complex health needs.
- Coordinate supports and collaborate with families, support workers, allied health professionals, and external stakeholders.
- Monitor participant outcomes and identify opportunities to improve service quality.
- Assist with risk assessments, incident management, and safeguarding practices.
- Maintain accurate documentation and clinical records.
- Support compliance with NDIS Practice Standards and Care Integrity policies.
- Advocate for participant choice, control, dignity, and independence.
Essential Requirements
- Current registration with AHPRA as a Registered Nurse.
- Experience working within the disability, community, aged care, or health sectors.
- Strong understanding of person-centered care principles.
- Excellent communication, relationship-building, and problem-solving skills.
- Ability to manage multiple participants and competing priorities.
- Current NDIS Worker Screening Check.
- National Police Check.
- Current First Aid and CPR Certification.
- Valid Driver's License and reliable vehicle.
- Full Australian working rights.
- Working With Children Check
Desirable Experience
- Experience working within the NDIS environment.
- Knowledge of NDIS funding, supports, and service delivery requirements.
- Experience supporting participants with complex needs and high-intensity supports.
- Care coordination, case management, or client relationship management experience.
