About the role
Reporting to the General Manager, the Community Leader is a Registered Nurse in a frontline clinical leadership role, responsible for the day-to-day operational and clinical leadership of a defined resident community within our Northshore Hamilton home.
You hold direct line management responsibility for the Senior Registered Nurses, Registered Nurses, Personal Care Workers, Dining Services Champion and Lifestyle Officers within your community, with Senior Registered Nurses acting as People Leaders for the direct care team and supporting you in the day-to-day leadership, development and performance of Personal Care Workers.
This role is centred on visible leadership, resident wellbeing and team culture, working in close partnership with the Clinical Nurse Consultant for specialist clinical advice, complex case support and escalation, and with the General Manager on broader operational and workforce matters.
Key responsibilities include:
- Provide visible, day-to-day clinical and operational leadership within your community, ensuring residents receive safe, dignified and genuinely person-centred care across all shifts
- Lead, supervise and develop your team including coaching, feedback and performance management
- Oversee the development, delivery and documentation of individualised care plans that are current, accurate and reflective of each resident's goals, preferences and clinical needs
- Monitor resident wellbeing on an ongoing basis, proactively identifying changes in condition, initiating timely clinical assessment and escalating complex or high-risk presentations to the Clinical Nurse Consultant
- Lead the operational rhythm of your community, including shift handovers, care team huddles, rostering and day-to-day coordination across all shifts
- Build strong, trusting relationships with residents and their families, responding promptly to feedback and concerns and facilitating meaningful family engagement
- Foster a team culture grounded in psychological safety, mutual accountability and continuous improvement, supporting the onboarding, orientation and development of new team members
Skills and experience
- Experience in residential aged care, with demonstrated ability to lead, supervise and develop a frontline care team including registered nurses and personal care workers
- Sound understanding of person-centred care principles and the obligations of the Aged Care Act 2024, Aged Care Rules 2025 and the Strengthened Aged Care Quality Standards
- Demonstrated experience in care planning, clinical documentation and clinical assessment within residential aged care or a comparable health setting
- Strong interpersonal and communication skills, with the ability to engage effectively with residents, families and multidisciplinary team members
- Demonstrated ability to manage competing priorities in a fast-paced frontline environment, maintaining calm, considered leadership under pressure
- Commitment to continuous improvement and a proactive approach to quality, safety and resident experience at the community level
- Resident-first, collaborative and values-led, bringing genuine enthusiasm, warmth and empathy to your work every day
- Visible people leadership, building a team culture where people feel safe, supported and accountable, and generous coaching and mentoring of nurses and carers
- Bachelor of Nursing (or equivalent) and current registration as a Registered Nurse with AHPRA, with no conditions or notations
- Postgraduate qualifications in aged care, leadership or a related specialty will be highly regarded (desirable)
