About the Role:
Lumiere Foundation is on the look out for an experienced NDIS Support Coordinator for work with ICCare's participants. You will provide varying levels of support coordination and transitions of supports for participants within their NDIS plan.
Essential Duties and Responsibilities:

Applies and adheres to NDIS and Child Safe standards and regulations
- Efficiently co-ordinate supports for participants within the funding levels allocated
- Liaise and work collaboratively with key stakeholders to establish a mutual goal of coordinating service delivery and developing effective linkages relevant to the achievement of the participant’s goals
- Empower clients to exercise choice and control to access a range of formal and informal mainstream and community-based support, by identifying opportunities for innovative service delivery
- Keep accurate records, files and reporting requirements in a consistent and clear manner that meet all guidelines, practice standards, and legislative requirements relating to the NDIS and the NDIS Quality and Safeguards
What You’ll Bring to the Role

Tertiary qualifications in social sciences, health or other relevant professional field or a diploma in Disability Services studies.
- At least two years’ experience in Support Coordination.
- Knowledge of the NDIS Quality and Safeguards Commission practice standards.
- Proficient in Microsoft Suite (Outlook, Word, Excel) and experience using a CMS system (MYP, Lumary, ShiftCare etc).
- Ability to speak Arabic is beneficial, but not necessary for the role.
- Excellent verbal and written communication skills in English
- Good report writing skills.

Unrestricted right to live and work in Australia.
- A current National Police Check.
- First Aid Certificate.
- Maintenance of a satisfactory Working with Children Check and NDIS Worker Screening Check.
- Full unrestricted NSW driver’s license and access to a reliable, roadworthy vehicle.
