About the Role:
In this dynamic position, you’ll provide comprehensive healthcare support to participants in line with NDIS guidelines. You’ll oversee clinical care, medication compliance, and risk mitigation while leading a team committed to delivering high-quality, participant-centered care. Key responsibilities include conducting health assessments, developing care plans, delivering training, and collaborating with internal and external stakeholders to achieve exceptional health outcomes.
Key Responsibilities:
- Conduct detailed health assessments and develop individualised care plans.
- Provide leadership to support workers, ensuring best practices in clinical care.
- Deliver health-related education and training to participants and caregivers
- Develop, implement, and regularly review individual care plans in collaboration with participants, their families, and Management.
- Monitor participant progress and compliance with NDIS standards.
- Respond to clinical incidents and manage risk mitigation strategies effectively.
- Collaborate with multidisciplinary teams to enhance care quality.

About You:
- Bachelor of Nursing - Registered Nurse with AHPRA registration.
- Minimum of 3 years’ experience in aged care or disability services.
- Strong leadership, communication, and interpersonal skills.
- Commitment to excellence in clinical care and continuous improvement.
- Valid First Aid/CPR, WWC, NDIS Worker Screening, Police Check and Australian Driver’s License.
- Availability - Monday to Friday 9am to 5pm.
- Available to work in both Salisbury and Prospect Offices.
