The Paediatric Comprehensive Care Liaison Nurse is a specialist nursing role responsible for coordinating and facilitating the care of children with complex and chronic health conditions—such as cystic fibrosis, renal failure awaiting transplant, and other multisystem disorders—who require multidisciplinary support and ongoing care after discharge from hospital. The role aims to ensure seamless, family-centred care by linking acute and community services, supporting discharge planning, and fostering strong collaboration between hospital teams, community providers, NDIS services, and allied health professionals.

Essential Requirements:
- Current registration as a Registered Nurse with AHPRA
- Ability to work independently across within AWH and with community services with strong organisational and project management skills.
- Current driver’s license and willingness to travel within the region when required.
- Complies with mandatory training and health service credentialing requirements.
- Minimum 5 years of paediatric nursing experience, including care of children with acute, chronic and complex conditions.
- Postgraduate qualifications in Paediatrics, Care Coordination, or Community Health.
