Social Worker – Allied Health (Veteran-Focused Care)
$75,000 -$80,000 per annum plus superannuation

Pivot Health Group is growing, and we’re excited to welcome a Social Worker to our innovative, multidisciplinary allied health team. Our clinics bring together Social Workers, Exercise Physiologists, Physiotherapists, Dietitians, Podiatrists, with GP services launching soon, to deliver truly integrated, client-centered care. We are proud to support a large cohort of veterans and have developed a distinctly unique model of care that prioritises collaboration, continuity, and holistic wellbeing.
This is a Full-Time position, working across our Rockingham, Mandurah, Port Kennedy and Baldivis Clinics.
About the Role
As a Social Worker at Pivot Health Group, you will play a vital role in supporting clients through their health journeys. You will work closely with clinicians across disciplines to enhance psychological wellbeing, social functioning, psychosocial engagement, and overall quality of life.
This role is ideal for a clinician who values teamwork, community engagement, and innovative service delivery.
Key Responsibilities
- Provide social work services to eligible clients in line with relevant frameworks, policies, and clinical standards.
- Deliver evidence-based assessments, interventions, and treatment plans
- Undertake psychosocial assessments, case formulation, treatment planning, and case management
- Coordinate care and facilitate internal and external referrals
- Conduct goal setting and regular client reviews in collaboration with clinic-based practitioners
- Provide generalist counselling addressing emotional, behavioural, and mental health challenges
- Support veterans in accordance with DVA guidelines
- Assist clients with documentation, forms, and applications for allied health services
- Facilitate and deliver social work programs and group activities (e.g. coffee catch-ups, walking groups, cinema outings)
- Conduct wellbeing check-ins and gym membership engagement calls
- Communicate effectively with allied health and medical professionals
- Provide health education and health promotion activities
- Maintain accurate clinical documentation, reporting, and consent processes
- Conduct online case conferences in collaboration with allied health professionals and general practitioners to support coordinated client care.

Essential Requirements
- Eligibility for membership with the Australian Association of Social Workers (AASW)
