The Role:
We are seeking a capable, organised, and compassionate NDIS Support Coordinator to join our team. This role involves working closely with NDIS participants, families, carers, service providers, and other stakeholders to help participants understand and implement their NDIS plans.
The successful candidate will support participants to build capacity, coordinate services, resolve barriers, and ensure supports are delivered in line with their goals, funding, and individual needs.

About the Role:
Your responsibilities will include:
- Supporting participants to understand and implement their NDIS plans
- Assisting participants to connect with suitable service providers and community supports
- Coordinating services across support workers, allied health professionals, plan managers, families, and other stakeholders
- Building participant capacity to exercise choice and control
- Monitoring participant goals, budgets, service agreements, and plan utilisation
- Assisting with service bookings, referrals, assessments, and reports
- Supporting participants with plan reassessments, change of circumstances, and evidence gathering
- Identifying and resolving barriers that may impact access to supports
- Maintaining accurate case notes, records, and documentation in line with NDIS requirements
- Working collaboratively with the Beyond Health Services team to ensure quality and consistent support
About You
You are a professional, empathetic, and highly organised individual with:
- Experience in NDIS Support Coordination, case management, disability, mental health, community services, or a related field
- Strong understanding of the NDIS, participant plans, funding categories, and service coordination
- Excellent communication and stakeholder management skills
- Ability to manage competing priorities and maintain accurate documentation
- A person-centred, respectful, and non-judgmental approach
- Strong problem-solving skills and the ability to advocate appropriately for participants
- Experience supporting participants with complex needs, psychosocial disability, or high support needs is highly regarded
NDIS Compliance
You will need the following, or be willing to obtain:
- NDIS Worker Screening Check
- Police Check
- Working with Children Check
- Current First Aid and CPR
- Relevant qualification in disability, community services, mental health, social work, case management, or similar is highly regarded
- ABN, if applying as a contractor
