Older Persons, Subacute and Rehabilitation Services (OPSAR) delivers integrated inpatient and community care focused on interprofessional collaboration, functional restoration, and management of complex health needs. Services support system navigation and coordinated care planning to optimise independence and quality of life. OPSAR includes: aged care assessment (FNQ ACAT, Memory Services), Transition Care Program (TCP), Commonwealth Home Support Program (CHSP), Neurology, OPERA and SACU subacute services, Rehabilitation, Palliative Care, Stroke, OPEN ARCH, GEDI, OPeLS, and Residential Aged Care Facility (RACF) In-Reach.

About your role:
Apply independent high level Social Work clinical decision making skills integrating evidence based practice to contribute to multidisciplinary complex case management and discharge planning to achieve best practice outcomes with infrequent need for direct clinical practice supervision.
Participate in the planning, development, delivery and evaluation of the Sub-acute services and social work services. Provide advice to operational and professional supervisors, relevant service managers and key stakeholders regarding service delivery and clinical service development at a local level.
Monitor and report on clinical practices and outcomes within the Sub-acute services. Contribute to the identification, development, implementation and evaluation of quality improvement and research activities.
Fulfil organisational and professional requirements for documentation, statistical records, data collection, meeting attendance and active participation, maintaining portfolios and adherence to protocols and procedures.
