About the role:
The Care Planner is responsible for coordinating and managing individualized care plans to ensure clients receive appropriate support and services. This role involves assessing client needs, collaborating with multidisciplinary teams, monitoring care outcomes, and maintaining accurate documentation.
Key Responsibilities
- Conduct client assessments and identify care needs.
- Develop, implement, and review personalized care plans.
- Coordinate services with healthcare providers, caregivers, and community resources.
- Monitor client progress and update care plans as needed.
- Maintain accurate records and documentation.
- Provide guidance and support to clients and families.
- Ensure compliance with organizational policies and regulatory requirements.
- Participate in case conferences and multidisciplinary meetings.
Essential Requirements
- Bachelor's degree in Social Work, Healthcare Administration, Nursing, Psychology, or a related field.
- Experience in care coordination, case management, or healthcare services preferred.
- Well-developed oral communication and information system usage skills.
- Exceptional customer service, negotiation, and influencing skills.
- Strong interpersonal and client service skills with high levels of resilience.
- Solid organizational skills including problem-solving, prioritizing, and multi-tasking.
- Good computer skills
- Group facilitation and presentation skills.
- Ability to work with diverse lifestyles and work styles.
- Staff management and client intake/care planning experience.
- Supportive and cooperative interaction approach.
- Focus on participant needs and quality service.
- Current driver’s license.
- First Aid/CPR, NDIS Worker’s Screening Card, Blue Card, and Police Check.
