iCARE Youth and Community Services Pty Ltd is a dedicated NDIS (National Disability Insurance Scheme) registered provider, and a provider of residential placements for the Department of Children and Families in the Northern Territory (NT). Our core mission is to provide essential care and support services to individuals with disabilities, ensuring their well-being, independence, and inclusion in the community. Our service offerings encompass a wide range of NDIS-approved supports, including accommodation, therapy, and community access programs, tailored to meet the unique needs of each individual we serve.
Due to increased demand for our services, we have multiple positions available for this role in Alice Springs and the surrounding areas of Central Australia, NT.
Position Description
Facilitate community development initiatives and collective solutions within a community to address issues, needs and problems associated with recreational, health, housing, employment and other welfare matters.
Duties:
- Assesses community needs and resources for health, welfare, housing, employment, training and other facilities and services
- Coordinate outreach and community participation supports for vulnerable participants across remote Northern Territory communities.
- Support socially isolated participants with transport, appointments, skill-building, tenancy maintenance, and community access.
- Identify and provide resolutions to immediate issues such as lack of housing, mental health crises and other health emergencies
- Assist individuals and families to be actively involved in their local community and engage in meaningful opportunities to promote healthy active living
- Provide practical and emotional support to individuals and families, and referrals to available resources and emergency supplies, such as food vouchers
- Assess individuals' and families' needs by planning, implementing and assessing relevant educational, training and support programs
- Raises awareness in the community and advocates for groups of people that require additional assistance to establish community connectivity
- Educates individuals to gain life skills to enable independent living and informs them of available support resources
- Identify individuals who require additional assistance and refer them to relevant agencies for specialised support.
- Supervise offenders on probation and parole
- Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
- Assisting young people to solve social, emotional and financial problems
- Preparing submissions for funding and resources, and reports to government bodies and other agencies
- Coordinates mobile voluntary services such as emergency food response or delivery of food for community programs
- Provides individuals with transport to appointments, programs or recreational activities
- Supports individuals and families with recovery planning, goal setting and access to mental health services
Eligibility requirements:
To be eligible, the ideal candidate must have/be willing to obtain the following:
Hold a relevant qualification equivalent to an AQF diploma and one year of full-time (or part-time equivalent) work experience or
At least 3 years of relevant full-time (or part-time equivalent) work experience
Current National Police Check
NDIS Worker Screening Check
First Aid Certificate
Availability to work across a 24/7 Roster to meet operational needs
NT Driver's Licence
