Job Role: The position is responsible for working in conjunction with a small team in the delivery of culturally appropriate mental health services to the community members living with mental health, disability and other comorbidities. This position will work to build capacity of older Australians living with mental health to provide recovery-centered programs and intervention for clients.
Job Duties:
- Deliver professional, person-centered care and positive behavior support across all shifts.
- Ensure clients are provided with strong and positive community integration program as well as social activities that stimulate their recovery
- Ensure integrated service delivery and follow up, coordinating and communicating supports needs as required with other relevant internal, visiting and external providers.
- Identify the health needs of clients and determine the appropriate nursing care to address priority needs/problems
- Perform age specific safety measures in all aspects of resident care
- Manage and administer wound care and monitor progress as per guidelines
- Support clients with skills development, transport, community activities, social support, recreational activities, daily cooking and cleaning tasks. Provide high quality support according to company standards to our clients and ensure that they achieve their full potential.
- Understand and acknowledge the company duty of care and duty of confidentiality to ensure that all support and information is confidential.
- Undertake relevant administration duties and provide the company with accurate reports such as incident reports, medication charts, injury reports, communication book notes, case notes etc. Ensure that all paperwork and information is recorded accurately and in a timely manner according to company standards.
- Work within a legal and ethical framework.
- Be proactive in-service delivery that promotes recovery and focus on client strengths.
- Ensure that clients activities of daily living are delivered with the utmost care and respect.
- Work professionally and sensitively with the client and their family, community and support network.
- Support clients and their best interest to make choices about their present and future in regards to education, health, careers, medication and other social situations.
- Organize and participate in appointments and activities with clients by working directly with them and their personal issues and needs.
- Listen to clients, help them establish goals, mentor them and always support them in their best interest.
- Ensure hygienic practices according to work health and safety standards.
- Follow all reasonable directions from senior staff and model a strong work ethic.
- Be ‘hands on’ and involved in direct service delivery.
- Attend all appropriate trainings and meetings that are relevant to client’s care.
- Attend to all challenges in a timely manner.
- Perform wound dressing and/or other nursing related procedures as required for clients.
- Observe, annotate and monitor client medication and report to higher delegate when necessary.
Mandatory Knowledge and Skills:
- Must be fully vaccinated against COVID-19 as per SA Health guidelines for the disability sector
- Punctual, honest and hard working.
- Strong written and verbal communication skills, excellent interpersonal skills and a strong ability to solve problems and de-escalate crisis situations.
- Ability to understand and adapt to different clients' processes
Desired Knowledge and Skills:
- Ability and experience working independently
- Knowledge of the Aged Care Accreditation Standards and ACFI
- Strong knowledge of NDIS Practice Standards
- Experience working with clients who have complex medical and/or mental health needs.
- Exceptional knowledge in medication and dietary management.
- Experience in manual handling, mobility assistance and personal care.
- Solid clinical assessment skills
Qualifications:
- Approved tertiary qualifications (Diploma or Degree in Nursing)
- Unrestricted registration with AHPRA
- Valid NDIS Worker’s Clearance and Working with Children Check
- Valid National Police Clearance (less than 1 year)
- Valid Driver’s License
- Full Comprehensive Car Insurance
- Medication Training
- Valid First Aid & Manual Handling Certificates
- Valid Working Rights in Australia
Pay: $35.00 – $45.00 per hour
