EXPERIENCED DISABILITY SUPPORT WORKERS – HIGH-NEEDS PERSONAL CARE

Ongoing Regular Shifts | 24-Hour, 7-Day Roster
Clearhaven Support Services is seeking experienced and dependable Disability Support Workers to provide individualised support to a participant with high support needs in a private residence in the Rockdale area.
This is an opportunity to join a small, participant focused team and provide consistent one to one support.
Regular ongoing shifts will be available across a seven day roster, including morning, afternoon, evening, weekend and public holiday shifts.
ABOUT THE ROLE
The successful applicants will support the participant to live safely, comfortably and with dignity in their own home.
The role requires a high level of reliability, professionalism, patience and attention to detail.
Responsibilities may include:
· Providing high quality personal care and daily living assistance including community participation
· Assisting with showering, dressing, grooming and personal hygiene
· Providing toileting and continence support
· Assisting with transfers, positioning and mobility
· Safely using mobility and manual handling equipment
· Supporting meal preparation, eating and drinking
· Providing medication assistance in accordance with the participant’s support plan and organisational procedures
· Monitoring the participant’s wellbeing and promptly reporting any concerns
· Following the participant’s support plan, health plans, risk assessments and established routines
· Completing accurate progress notes, medication records and incident documentation
· Maintaining a safe, clean and comfortable home environment
· Supporting the participant’s independence, preferences, privacy and personal choices
· Communicating professionally with the participant, family members, other support workers and relevant health professionals
ABOUT YOU
We are looking for experienced Support Workers who understand that consistency and trust are essential when supporting a person with high needs. You must be comfortable providing personal care and working independently within a participant’s home.
To be considered, applicants must have:
· Certificate III in Individual Support (Disability), Community Services or an equivalent relevant qualification
· Demonstrated experience supporting people with high personal care or complex support needs
· A current NDIS Worker Screening Check
· A current National Police Check
· Completion of the NDIS Worker Orientation Module
· Evidence of relevant infection prevention and control training
· The right to work in Australia
· A current Australian driver licence and access to reliable transport
· The physical capacity to safely undertake personal care, mobility assistance and manual-handling duties
Additional training or demonstrated competency in high intensity daily personal activities, medication assistance, manual handling, mealtime support or continence care will be highly regarded.
