Fractional Specialist - Hospital Without Walls (HWoW)

About the role
We are seeking a Fractional Specialist Physician or General Practitioner to deliver specialist geriatric and complex care to patients across the Grampians region — both in hospital and in the community. Reporting to the Clinical Directors, Hospital Without Walls and working as a senior member of the medical team, you will provide leadership and clinical expertise in a team-based environment.
In this role you will:
- Provide specialist clinical services to older patients with complex care needs, including direct and remote (telehealth) assessment, complex management planning, prescribing and collaborative team decision-making.
- Lead and support the multidisciplinary team to achieve clearly documented patient and team goals, and coordinate care with nursing, allied health, diagnostic and community services.
- Supervise, teach and mentor junior medical staff allocated to HWoW and Geriatric Medicine.
- Work closely with clinicians across the region to develop and deliver effective community-based services.
- Contribute to education, collaborative strategic planning, policy and procedure development, quality improvement and patient consultation.
- Ensure high-quality, complete documentation of patient care and evidence-based practice always.

About you
To be successful in this role you will have:
- MBBS or equivalent degree, with current medical registration in Australia.
- Fellowship of the Royal Australasian College of Physicians (FRACP) or the Royal Australian College of General Practitioners (FRACGP), or an equivalent qualification recognised by the Royal Colleges.
- Demonstrated experience and skills in the medical assessment and clinical management of complex older patients.
- Detailed knowledge of the investigation and management of acute and chronic medical disorders, and an understanding of the impact of illness, disability and ageing on patients and their families.
- The ability to lead a multidisciplinary team and operate effectively at a senior clinical level.
- Excellent written and verbal communication skills, and the ability to work effectively both independently and as part of a team.
- A commitment to quality improvement, ongoing professional development and evidence-based practice.
