Brother Bear Support Services Pty Ltd is seeking a motivated and compassionate Community Worker to join our growing NDIS team on a full-time basis in Thomastown, Victoria.
The successful applicant will work collaboratively with participants, families and service providers to promote independence, community inclusion and capacity building through person centered planning and coordinated supports.
Key Responsibilities
- Assess participant needs and assist in developing individual support and community participation plans.
- Coordinate access to community services, recreational activities and support programs.
- Liaise with participants, families, carers, allied health professionals and external agencies.
- Support participants to achieve greater independence, social inclusion and community engagement.
- Assist with referrals, service coordination and implementation of participant goals.
- Promote capacity building, choice and control through person-centred approaches.
- Monitor participant progress and maintain accurate documentation and records.
- Work collaboratively with the wider team to achieve positive participant outcomes.
Essential Requirements
- Certificate III and/or Certificate IV in Individual Support, Community Services or a related qualification.
- Minimum 1 year experience in the disability or community services sector.
- Current NDIS Worker Screening Check.
- Current Working with Children Check.
- Current Australian Driver Licence.
- Excellent communication and interpersonal skills.
- Ability to work independently and within a multidisciplinary team.
Employment Type: Full-Time
Location: Thomastown, Victoria
Salary: $80,000–$85,000 per annum plus superannuation, depending on qualifications and experience.
Pay: $80,000.00 – $85,000.00 per year
