Qualifications & experience:
Previous experience in a similar role – Minimum of 2-year relevant experience Previous experience supporting People with Disability.
Demonstrated proficient written, verbal and interpersonal communication skills. Demonstrated ability to work with minimal supervision, and collaboratively as a member of a multidisciplinary team.
Prevoius experience in mental health and Pysical disability such as ASD, ADHD and lots more.
Demonstrate ability use manual handling such as hoisting, peg feeding and lots more.
Current National Police Check, Working with Children Check. First Aid certificate and CPR, NDIS Workers Orientation and 100 Points of ID.
Tasks & responsibilities:
Support clients to participate in age-appropriate activities and tasks including employment, in home support and community access.
Support clients to access activities and outings in the community. Provide direct assistance to each client in accordance with their service plan and as directed.
Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality. Monitor the safety and well-being of the clients and report any concerns to the Manager.
Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.

Pay: $40.15 – $70.33 per hour
