Behind every client living independently at home is a care plan that actually works — and someone making sure it does. That someone is you.
As a Support at Home Coordinator with our Veterans and Aged Community Care team, you'll guide clients through their Support at Home journey from start to finish. Understanding their goals, connecting the right services, keeping families in the loop, and making sure nothing falls through the cracks. It's detail-oriented, it's relationship-driven, and when you get it right, you see it — in the confidence of a client who feels genuinely supported.
What you'll be doing
- Managing Support at Home services end-to-end — onboarding, reviews, service changes — with care, accuracy and follow-through
- Helping clients understand what their package can do for them and building plans that reflect what actually matters to them
- Being the consistent, trusted point of contact for clients and families — the voice they're glad to hear
- Keeping documentation tight and flagging anything important to the Senior Clinical Nurse before it becomes a problem
- Responding to feedback and incidents with speed and professionalism, and feeding insights back into continuous improvement
- Contributing to the growth of the Support at Home program as Kanda continues to expand
You're the right person if...
- You have solid experience in aged care, community care or disability services in a coordination or case management role
- You hold a relevant qualification in Health or Community Services — or equivalent hands-on experience
- You communicate with warmth and clarity across every level — clients, families, field staff and clinical leads
- You're organised enough to manage competing priorities without letting the important things slip
- You hold a Working with Children Check and NDIS Worker Screening Clearance, or you're ready to get one
