Booroongen Djugun Limited (BDL) is an Aboriginal Community-Controlled Organisation. BDL was founded in consultation with traditional Elders and designed to reflect the emotional, spiritual and physical needs connected with the traditions and customs of Aboriginal & Torres Strait Islander people. BDL continues to operate in alignment with these core foundations to this day, providing cultural safe services and employment for all Aboriginal & Torres Strait Islander Residents, Staff, Clients and Community.
All BDL Staff are to have sound knowledge and understanding of applying culturally safe practices during their employment at BDL, or the capability to seek assistance and guidance on how to apply culturally safe practices.
The Community Care & Service Delivery Coordinator is responsible for leading and coordinating the day-to-day operations of the Community Care & Service Delivery (CCSD) Team, to deliver high quality services across the NDIS and Aged Care Programs. This position will coordinate culturally safe, person-centred service delivery, ensuring alignment with the NDIS Quality & Safeguards Commission’s (NDISQSC) Practice Standards, Aged Care Quality & Safety Commission’s (ACQSC) Safey Standards, community values, funding requirements, and sector reforms while supporting the empowerment of Aboriginal & Torres Strait Islander people through high-quality, holistic care.
Service Delivery

Lead and coordinate operations of the Community Care and Service Delivery Team, ensuring effective delivery of services NDIS and Aged Care programs in a culturally safe environment including client intake, staff rostering, compliance, incident management and employee relations, in a culturally safe environment.

Ensure the delivery of all support services are in line with BDL’s Participants needs and goals.

In collaboration with the CCSD Manager ensure all participant documentation, such as rosters of care, Individual Support Plants, Schedules of Supports and Service Agreements are accurate, compliant and safeguarded.

In colloboration with the CCSD Manager and BSU Team ensure all client services are monitored in alignment with financial capabiliites, service utilisation, service delivery, client budget needs are met and on target.

Enbed and practice cultural safety and community engagement in all aspects of service delivery.

Build and maintain strong relationships with internal & external stakeholders, BDL’s NDIS and Aged Care Participants, families and caregivers, addressing concerns and enquiries within required timeframes.
- Collaborate with the Leadership Team to deliver consistent, safe, and high quality support services, system improvements and growth of the CCSD Team.
Leadership & Management

Support and supervise the CCSD Team, demonstrating positive work behaviours and using strong interpersonal skills in an engaging manner to motivate and influence others.

Work collaboratively across the organisation and participate in the Leadership Team, sharing knowledge and experiences.
- Contribute to workforce development through recruiting, training, performance management in collaboration the CCSD Manager and HR Advisor, and assist with staff initiatives.

Ensure that key responsibilities are carried out in an efficient and conscientious manner within set timeframes, deadlines and budget constraints.
- Monitor and ensure adherence to the SCHADS and Fair Work standards.
Support the continuous development of team members in collaboration with the CCSD Manager by maintaining performance and development plans to achieve optimum productivity.

Selection Criteria
To be considered for this position, applicants must meet all areas of the selection criteria, providing a covering letter addressing each of the below:

Aboriginal and Torres Strait Islander people strongly desired and/or experience and knowledge working with Aboriginal & Torres Strait Islander people, including people from Aged Care, Disability and comorbidity complex environments.

Strong understanding of Aboriginal & Torres Strait Islander community, and a sound understanding of the issues affecting Aboriginal & Torres Strait Islander people, families and communities.

Demonstrate experience at a management level, dedicated to delivering excellence in culturally safe person-centred care across the current and future needs of Aboriginal & Torres Strait Islander with a commitment to Closing the Gap (CtG).
- Tertiary qualifications in Allied Health, Human Services (Aged Care and Disability Sectors), Compliance or a relevant discipline, or demonstrated experience in conjunction with tertiary qualifications.

Demonstration of strong, motivating leadership skills, with experience of change management, overseeing cross functional teams, coordinating resources to ensure delivery within a set timeframe and budget.

Sound understanding of compliance and regulatory requirements aligned with Age Care Quality Standards, NDIS Practice Standards and ASQA Standards and experience ensuring organisational compliance with relevant legislation, standards and frameworks.

Exhibit accountability, professional integrity and strong communication skills to manage internal and external stakeholder relationships effectively and positively influence outcomes through those relationships.
