Role description
We are seeking a motivated and compassionate Social Worker to join our team at Dhelkaya Health.
In this role, you will deliver person-centred social work services across a range of clinical areas, with a primary focus on inpatient units including Acute, Rehabilitation, Transition Care Program (TCP), and Residential Aged Care across our Castlemaine and Maldon campuses. The position also includes supporting clients within their homes in the community.
You will work as part of a multidisciplinary team, collaborating closely with nursing, medical, and allied health professionals to achieve the best possible outcomes for clients, their families, and carers. The role also involves building strong connections with internal teams and external community service providers to ensure coordinated, holistic care.
This is a dynamic role, and you may be required to work across different service areas to support organisational needs and ensure continuity of social work services.
Key Responsibilities

Deliver holistic, person-centred social work services using a Social Model of Health approach

Assess client needs and develop, implement, and review tailored care plans aligned with client goals and priorities

Provide evidence-informed interventions in line with best practice standards

Facilitate collaborative multidisciplinary care through active participation in case conferences

Provide short-term counselling to clients experiencing a range of needs, including chronic disease, rehabilitation, and mental health concerns

Advocate for clients, carers, and families to support positive outcomes
-
Coordinate and lead family meetings to support safe, effective discharge planning
-
Build strong working relationships with colleagues, community providers, clients, and families

Deliver education sessions to individuals or groups as required

Maintain accurate, timely clinical documentation and data reporting

Contribute to service development, evaluation, and continuous improvement initiatives

Work in alignment with organisational goals, contributing to service sustainability

Deliver services within funding parameters and resource constraints

Contribute to a positive, supportive, and collaborative team environment
