This is a role for a nurse who wants to make a real impact, guiding clients to live safely, independently and with dignity in their own homes.
About the Role
As a Clinical Care Partner, you will provide clinical leadership, care coordination and oversight for clients receiving services through the Support at Home Program. You’ll manage your own caseload, conduct clinical assessments, develop care plans, support care staff, and ensure services align with each client’s classification, goals and available budget.
You’ll be a trusted clinical contact for clients, families, care workers and multidisciplinary partners ensuring care is appropriate, safe, coordinated and person‑centred.
Key Responsibilities
· Support at Home Client and budget management
· Clinical assessment and care planning
· Clinical leadership and care staff oversight
· Support at Home pathways management and coordination
· Clinical governance and quality management to ensure client outcomes.
Essential Requirements
- Current AHPRA Registered Nurse registration
- Experience in community nursing, aged care, rehab or palliative care
- Strong clinical assessment and care planning capability
- Understanding of Support at Home classifications, funding and service delivery
- Experience supervising or guiding care staff
- Current First Aid & CPR
- Driver’s licence and reliable vehicle
- Police and screening clearance
- Ability to work independently and travel to clients’ homes.
