We are looking for an enthusiastic and motivated Chronic Disease Management Nurse to join our growing multidisciplinary team in a role dedicated exclusively to Chronic Disease Management.
This is an exciting opportunity for a nurse who is passionate about preventative healthcare, enjoys building meaningful relationships with patients, and thrives on helping people better understand and manage their health.
You'll work closely with our GPs to deliver high-quality Chronic Disease Management services, supporting patients living with chronic and complex conditions while empowering them to achieve better health outcomes.
Our administration team will manage patient recalls and appointment bookings, allowing you to focus on what you do best—providing exceptional patient care.
Your Role
You will be responsible for conducting:
- GP Management Plans (Care Plans)
- Team Care Arrangements
- Chronic Disease Management Reviews
- Medicare Health Assessments
- Coordination of referrals to allied health providers
- Patient education and preventative health support
- Accurate clinical documentation using Best Practice software

This is a dedicated, full-time position (Monday to Friday) with a consistently booked appointment schedule. We are looking for someone who enjoys working in a structured environment, maintains excellent time management skills, and is committed to achieving both clinical excellence and practice performance goals.
Essential Criteria
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Experience preparing Chronic Disease Management Plans and Medicare Health Assessments
- Excellent communication and patient education skills
- Strong organisational and documentation skills
- Experience using Best Practice software
Highly Desirable
- Previous experience in a dedicated Chronic Disease Management role
- Previous experience working in General Practice
- Knowledge of Medicare Chronic Disease Management item numbers
- Experience with Primary Sense
- Experience with HotDoc
