Guardian Nursing Agency is seeking a compassionate and experienced Registered Nurse to join our growing community care team in Kalgoorlie, WA.
This role provides high-quality nursing care to older Australians in their own homes, supporting them to remain safe, healthy and independent within their communities.
You will undertake clinical assessments, care planning, health monitoring and care coordination while working with clients who may have complex medical conditions and age-related health needs.
Key Responsibilities
- Conduct comprehensive nursing assessments in clients' homes.
- Develop, implement and review individualised care plans.
- Monitor chronic health conditions and changes in clients' health status.
- Undertake health and wellbeing checks.
- Support medication management and provide medication education.
- Provide wound care, continence management and chronic disease management.
- Coordinate referrals to GPs, allied health professionals and specialist services.
- Support falls prevention, mobility and assistive technology needs.
- Work collaboratively with Home Care Package coordinators and multidisciplinary teams.
- Educate clients and carers regarding disease management, medication compliance and healthy ageing.
- Promote independence and self-management.
- Maintain accurate clinical documentation, risk assessments and incident reports.
- Participate in quality improvement activities and clinical audits.
- Ensure compliance with relevant aged care, nursing and organisational requirements.
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse.
- Minimum 2 years' post-registration experience.
- Experience in community nursing, aged care, disability care or chronic disease management.
- Strong clinical assessment and care planning skills.
- Understanding of aged care and person-centred practice.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and manage a community caseload.
- Current National Police Clearance.
- Current Working with Children Check, or willingness to obtain.
- Current First Aid and CPR certification.
- Current unrestricted Australian Driver's Licence.

- Diabetes and cardiovascular disease.
- COPD.
- Mobility impairment and falls risk.
- Mental health conditions.
- Assistive technology and mobility equipment.
