About the role
The Clinical Nurse Consultant - Discharge Support Service provides advanced clinical expertise and leadership to support patients experiencing concerns or complications following medical or surgical discharge from Caboolture Hospital. The role is integral to the delivery of a new service designed to improve continuity of care, streamline patient journeys and reduce avoidable presentations to Emergency Departments through the provision of timely, patient-centred support.
The successful candidate will act as a clinical expert, mentor and advisor within the Discharge Support Service, providing specialised assessment, education and support to patients following discharge. The role works collaboratively across medical, surgical and multidisciplinary teams to drive service innovation, improve patient outcomes and ensure high-quality, evidence-based care is delivered in the most appropriate setting.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Provide advanced clinical expertise and specialised nursing support for patients requiring follow-up care and intervention following hospital discharge.
Demonstrate expert clinical judgement, critical thinking and problem-solving skills to manage complex discharge support issues and patient care needs.
Act as a clinical resource, mentor, educator and advisor to support staff and promote evidence-based practice across the service.
Collaborate effectively with multidisciplinary teams to improve continuity of care, support service innovation and enhance patient outcomes.
Contribute to policy development, quality improvement initiatives, service evaluation and the implementation of best practice discharge support processes.
