Community Housing Limited is seeking 2 x Initial Assessment & Planning Workers (Corrections) to join the Corrections Housing Pathways Initial Assessment and Planning Service. Reporting to the Regional Homelessness Services Coordinator, you will provide person-centred housing assessment, planning and referral support to individuals at Fulham Correctional Centre who are at risk of homelessness or unable to sustain their current tenancy upon release.
We are recruiting for 2 x permanent roles (1 x full time and 1 x part time working 5 days a fortnight). These roles can be based in either our Bairnsdale or Morwell offices, and there is a requirement to work at the Fulham Correctional Centre when needed. A driving license is required to travel to the Fulham Correctional Centre, and a pool car will be provided.

What you’ll do
In this role, you’ll assess housing needs, coordinate appropriate responses and support safe, stable housing outcomes before and after release from custody. You’ll work respectfully and confidentially with clients, Corrections Victoria, DFFH, real estate agents, VCAT and partner agencies to identify housing pathways, reduce the risk of homelessness and connect people with the right support at the right time.
You’ll make an impact by:

Completing housing assessments and initial planning with people in custody who are at risk of homelessness or tenancy breakdown.

Supporting clients to access appropriate housing pathways before and following release.

Providing information, advocacy, referrals and practical support in a respectful, confidential and client-centred way.

Distributing financial assistance in line with program guidelines and funding conditions.

Advocating for clients, including attendance at VCAT where required.

Building and maintaining effective relationships with partner agencies, including Corrections Victoria, DFFH, real estate agents and community support services.

Identifying and managing risks for yourself and clients while contributing to a safe, respectful and compliant working environment.

Maintaining accurate records, data entry and reporting, and supporting the wider team and organisation as needed.
What You’ll Bring
-
A qualification in Social Work, community services or another relevant social discipline.

Demonstrated understanding of issues affecting socially disadvantaged people, including people in custody or transitioning from custody.

Awareness of, and sensitivity to, the needs of people experiencing housing crisis, homelessness, disability or complex support needs.

Strong assessment, referral, advocacy, stakeholder engagement, problem-solving and organisational skills.

Ability to work collaboratively with Corrections Victoria, DFFH, real estate agents, VCAT, community services and internal CHL teams.

A positive and balanced approach to work, with a commitment to CHL’s values, safety and a respectful, inclusive workplace culture.

A current Driver Licence and ability to meet Corrections Victoria security clearance requirements.
Appointments are subject to satisfactory pre-employment checks, including National Police Check, Working with Children Check, NDIS Worker Screening Check and any security clearance requirements of Corrections Victoria. All pre-employment checks will be arranged during onboarding.
