What you’ll be doing
As an Aged Care Assessor, you will conduct face-to-face, in-home assessments using the Integrated Assessment Tool (IAT) to accurately identify the needs of individuals seeking home support services. Drawing on your assessments, you will develop personalised support plans and coordinate access to appropriate services, empowering customers to maintain their independence and quality of life. Whether it’s identifying respite options, developing emergency care plans, or linking people to short-term support, you’ll help turn something that feels overwhelming into something people can actually navigate.
In this role, you will:
- Work collaboratively with customers, carers, families, and service providers to support the aged care planning process
- Develop and review personalised support plans, ensuring they reflect changing needs and align with assessment outcomes
- Identify existing supports, including formal and informal respite arrangements, and develop Emergency Care Plans where required
- Recognise when more complex or higher-level assessments are needed and escalate appropriately
- Assess risk, vulnerability, and complexity to connect customers with appropriate services, including reablement and short-term supports
- Complete administrative tasks such as report writing and correspondence, ensuring accurate and timely documentation
This is a Permanent, Full-Time opportunity where you’ll travel across Hervey Bay and Maryborough region to engage directly with customers and key stakeholders.
You’ll thrive in this role if you

- Have experience conducting home or risk assessments, especially for vulnerable people
- Communicate clearly and kindly, even with customers who have hearing loss, cognitive impairment, or dementia
- Understand the impact of ageing and disability on individuals and families
- Are confident using technology and open to learning new systems
- Know how to work independently while staying connected to your team
- Bring cultural awareness and trauma-informed care to every conversation
- Hold a current First Aid & CPR certificate, driver’s licence, and are willing to use your own vehicle for work
