We are currently seeking a Care Designer to join our team in a permanent part-time role. This position would suit someone who is highly organised, confident communicating with participants and families, and able to balance care planning, rostering, staffing and general enquiries.
This role will commence at 15 hours per week, working 3 hours per day, Monday to Friday, with the potential to increase as the company grows.
About the Role
The Care Designer will support the intake, onboarding, care planning and rostering process for new and existing participants. You will work closely with participants, families, support coordinators and support workers to help ensure supports are planned, staffed and delivered smoothly.
This role will be completed under the oversight of the Business Manager, with clear processes, templates and guidance provided.
The position will include a mix of office-based administration, phone and email communication, rostering, and face-to-face care planning meetings with participants across the Gold Coast.
Key Responsibilities
Your duties will include:
Client Intake and Onboarding
- Managing new participant leads and enquiries
- Following up email and phone enquiries
- Arranging and attending face-to-face care planning meetings with participants across the Gold Coast
- Gathering relevant information about participant goals, routines, preferences, risks and support needs
- Preparing care plan reports and onboarding documentation under the oversight of the Business Manager
- Assisting with the design of supports, including community access, in-home support and respite arrangements
Rostering and Scheduling
- Creating and maintaining participant rosters
- Arranging replacement staff when support workers are sick or unavailable
- Communicating roster updates with participants, families and support workers
- Monitoring recurring shifts and completing quarterly check-ins to ensure supports are still meeting participant needs
- Identifying gaps in the roster and escalating staffing needs
Staffing Support
- Assisting with workforce planning to ensure there are sufficient support workers available for current and upcoming rosters
- Conducting first-stage interviews with potential support workers
- Supporting the recruitment process by identifying suitable candidates for participant needs
- Communicating staffing requirements to the Business Manager
General Enquiries and Respite Planning
- Responding to general enquiries from participants, families and support coordinators
- Assisting with staffing and planning for respite requests
- Gathering information required to quote, design and organise short-term support arrangements
- Following up enquiries in a timely and professional manner
- Highly organised and able to manage competing priorities
About You
We are looking for someone who is:
- Highly organised and able to manage competing priorities
- Confident speaking with participants, families, support coordinators and support workers
- Professional, warm and clear in written and verbal communication
- Comfortable working independently while following established processes
- Able to problem-solve when shifts need replacing or support arrangements change
- Respectful, person-centred and attentive to participant choice and control
- Confident using computers, email, online systems and rostering platforms
- Able to attend care planning meetings across the Gold Coast
Experience in disability, aged care, support coordination, rostering, case management, intake, administration or recruitment would be highly regarded.
Essential Requirements
- Current NDIS Worker Screening Check
- Current Working With Children Check
- Current Australian driver licence
- Reliable vehicle with comprehensive insurance
- Strong computer and administration skills
- Availability to work Monday to Friday mornings
Desirable Experience
- Experience in the NDIS or disability support sector
- Rostering or scheduling experience
- Understanding of participant-centred practice and duty of care
- Experience working with support workers, families or support coordinators
- Experience completing intake or care planning documentation
- Recruitment or interviewing experience
