About the role:
As the Healthy Ageing Team Leader, you will be responsible for providing day-to-day operational leadership for the Healthy Ageing services across the central region, including the Commonwealth Home Support Programme (CHSP) and other aged care programs.
You will drive service excellence, workforce capability, operational performance and continuous improvement whilst ensuring services are delivered in accordance with contractual obligations, quality standards and organisational priorities. Working collaboratively with clinical, operational and sector leadership teams, you will support the ongoing development of responsive, sustainable and person-centred aged care services that deliver positive outcomes for consumers and the organisation.
The role ensures comprehensive, timely, and accurate compliance with reporting requirements, including Data Exchange (DEX), to guarantee funding deliverables are met and revenue is secured.
You will bring:

Significant experience in leading teams within the aged care or community services sector, specifically Support at Home and CHSP.

Demonstrated experience managing service delivery targets, operational performance (e.g. workforce and budget management) and supporting quality improvement, compliance and accreditation activities.

Experience managing incidents, complaints and service escalations in a regulated environment.

Strong understanding of the Aged Care Quality Standards, relevant funding legislation and sector reform priorities.

Demonstrated ability to lead, coach and develop diverse multi-disciplinary teams to achieve high performance and positive outcomes.

Experience using service data and operational reporting to inform decision-making to deliver workforce efficiency, and improved outcomes.

Highly developed communication, negotiation and relationship building skills.

Excellent written and verbal communication skills, including the preparation of high-quality operational reports, financial reporting and presentations.

Proficiency in Microsoft Office Suite and relevant client management/operational software systems.

Ability to build relationships with consumers, carers, community organisations and internal stakeholders.

Experience in change management and organisational development (desirable).

Knowledge of contemporary support models, trauma-informed care and person-led practice (desirable).
Requirements:

Tertiary qualifications in Aged Care, Community Services, Social Work, Business Management, Health, or a related field.

Current QLD Driver’s Licence and capacity to travel.

NDIS Worker Screening (Yellow Card) and Blue Card.
