About the role
The Flexible Assertive Treatment Team (FACT) sits within the community of Orange and provide ongoing recovery oriented assessment and treatment through case management, aimed at improving the quality of life for older people with mental health needs living in the community..
FACT team services are delivered by a multidisciplinary team who provide ongoing recovery oriented assessment and treatment through case management, aimed at improving the quality of life for older people with mental health needs living in the community. With a focus on hospital avoidant community-based care and cooperative transfer of care for people leaving hospital into community mental health care. We provide patients a more flexible approach at times of need.
This is an exciting opportunity to be involved in a service model for WNSWLHD aimed at increasing the availability of clinical community-based support, changes in professional practice and most importantly, collaboration between all services.
The recovery-oriented approach is person centred, trauma informed and involves partnering with general health providers and agencies that can offer clients community living supports such as housing, disability services, vocational, social and educational opportunities.
