About the role
Permanent part time opportunity to provide diabetes and dietetic education and support to people living in Central Queensland with active diabetes-related foot disease in partnership with local and tertiary healthcare providers across Queensland Health.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Manage an allocated workload to deliver high level dietetic education to patients with diabetes and foot disease across the continuum of care, within a multidisciplinary team.
Provide clinical support and guidance to healthcare providers involved in the care of individuals with active foot disease in Central West, Central Queensland and Wide Bay, fostering a collaborative community of practice that promotes continuous education, enhances clinical skills, and supports sustainable, high-quality care delivery through capability building.
Monitor and report on clinical practices and outcomes within clinical area and actively participate in initiating, developing and implementing quality and service improvement activities that enhance the delivery of nutrition and dietetic services and patient outcomes.
Provide personalised diabetes education with focus on self-management including blood glucose monitoring, medication adherence, lifestyle modifications, managing diabetes-related complications and troubleshooting support for diabetes technology.
